Published online 2006 Jun 14. doi: 10.1186/1743-7075-3-23 PMCID: PMC1523335 state that An obese patient with type 2 diabetes whose diet was changed from the recommended high-carbohydrate, low-fat type to a low-carbohydrate diet showed a significant reduction in bodyweight, improved glycemic control and a reversal of a six year long decline of renal function. The reversal of the renal function was likely caused by both improved glycemic control and elimination of the patient’s obesity. Insulin treatment in type 2 diabetes patients usually leads to weight increase which may cause further injury to the kidney. Although other unknown metabolic mechanisms cannot be excluded, it is likely that the obesity caused by the combination of high-carbohydrate diet and insulin in this case contributed to the patient’s deteriorating kidney function. In such patients, where control of bodyweight and hyperglycemia is vital, a trial with a low-carbohydrate diet may be appropriate to avoid the risk of adding obesity-associated renal failure to already failing kidneys.
  3. Reversal of Diabetic Nephropathy by a Ketogenic Diet by Michal M. Poplawski, Jason W. Mastaitis, Fumiko Isoda, Fabrizio Grosjean, Feng Zheng, Charles V. Mobbs Published on April 20, 2011 in http://dx.doi.org/10.1371/journal.pone.0018604 conclude Intensive insulin therapy and protein restriction delay the development of nephropathy in a variety of conditions, but few interventions are known to reverse nephropathy. Having recently observed that the ketone 3-beta-hydroxybutyric acid (3-OHB) reduces molecular responses to glucose, we hypothesized that a ketogenic diet, which produces prolonged elevation of 3-OHB, may reverse pathological processes caused by diabetes. To address this hypothesis, we assessed if prolonged maintenance on a ketogenic diet would reverse nephropathy produced by diabetes. In mouse models for both Type 1 (Akita) and Type 2 (db/db) diabetes, diabetic nephropathy (as indicated by albuminuria) was allowed to develop, then half the mice were switched to a ketogenic diet. After 8 weeks on the diet, mice were sacrificed to assess gene expression and histology. Diabetic nephropathy, as indicated by albumin/creatinine ratios as well as expression of stress-induced genes, was completely reversed by 2 months maintenance on a ketogenic diet. However, histological evidence of nephropathy was only partly reversed. These studies demonstrate that diabetic nephropathy can be reversed by a relatively simple dietary intervention. Whether reduced glucose metabolism mediates the protective effects of the ketogenic diet remains to be determined.

The Fat in your body is not constant. It changed. It changes. And it will Change.  Change every day, every hour, every minute and every second. At any given moment, new fat is made and a part of fat gets degraded

Now image the overhead tank in your house. It has an inflow from Well or Underground Water Sump. And the overhead tank has an outflow of water to various rooms in your home

  • If the inflow into the tank matches the outflow, then the level of water will be constant.
  • If the inflow is greater than the outflow, then the level of water will increase.
  • If the inflow is lesser than the outflow, then the level of water will decrease.

Same way, Coming back to fat

  • If the amount of fat synthesized is equal to the amount of fat degraded, then the fat in your body will be constant
  • If the amount of fat being made is greater than the amount of fat being broken, then you will keep on gaining weight and become obese.
  • If the amount of fat being made is lesser than the amount of fat being broken, then you will keep on losing weight and become lean.

The Fats / Lipids in Body exist in two forms

  1. Element Constant of Structural Lipids. These form the various structures and are relatively constant. These are mainly phospholipids
  2. Element Variable or stored Lipids or Depot. These are mainly used for Fat Storage and the amount of this varies as per the food and need. These are Mainly Triglycerides. These are the ones which get deposited in your belly and gluteal region and in other regions making you obese

How are Triglycerides Made

Triglycerides are made from two components

  1. Free Fatty Acids : These are derived from the Fat as well as from Carbohydrates in the food
  2. Glycerol Phosphate : These are derived from the Carbohydrate in Food

So, to make Triglycerides, you need either Fat + Carbs or Carbs Alone. With Fat alone, little Triglycerides are synthesised. 

The process of synthesis of Triglycerides (aka Belly and Buttocks Fat) is supported by few hormones and opposed by few hormones

Insulin Supports this synthesis

How are Triglycerides Broken

Triglycerides are broken to Glycerol Phosphate and Free Fatty Acid by an enzyme called Hormone Sensitive Triglyceride Lipase. This action is inhibited by Insulin and favoured by

  1. Epinephrine
  2. Norepinephrine
  3. Glucagon
  4. Growth Hormone
  5. Glucocorticoids
  6. ACTH
  7. MSH
  8. TSH
  9. Vasopressin

Effect of Insulin

Insulin favours Formation of Triglycerides. Insulin Opposes Breaking down of Triglycerides. Insulin favours formation of adipose tissue and inhibits breakdown of adipose tissue. In Short, Insulin leads to a person becoming obese, gaining wait

What happens in Carbohydrate Rich Food

When the food is rich in Carbohydrates,

  1. More Glycerol Phosphate is formed. More Substances to form Fat
  2. More Free Fatty Acids are formed
  3. More Insulin is secreted by Pancreas. More Stimulus for Fat Formation. Less Stimulus for Fat Breakdown

All these three act to create more and more fat or adipose tissue and the person becomes obese, even if he does not take fat in his meal

In short, during Carbohydrate Rich Food, the rate of formation of fat is higher than the rate of destruction of fat, leading to net increase of fat in the body



What happens in Low Carbohydrate High Fat Food

When  the food does not have Carbohydrates or when the food has low Carbohydrates

  1. Less Glycerol Phosphate is formed. Hence there is less substance needed formation of fat
  2. Less Insulin Secretion. Hence less stimulation for Fat Formation
  3. Less Insulin Secretion. So there is more breakdown of fats by the hormones like Epinephrine, Norepinephrine, Glucagon, Growth Hormone, Glucocorticoids, ACTH, MSH, TSH, Vasopressin

So, in short, during a Low Carbohydrate diet, the formation of fat is very less, but destruction (degradation or lipolysis) of fat is accelerated leading to net loss of fat in the body. Hence people lose weight and also the fat stores in their abdominal wall and gluteal region begin to disappear

What happens to the Fat that is broken down

The Fat is broken down into Glycerol Phosphate and Free Fatty Acids. Of this

  1. Glycerol Phosphate can be converted to Dihydroxy acetone Phosphate and this can be converted to Glyceraldehyde Phosphate which is an intermediary in the breakdown of Glucose.  Hence this enters the Glycolytic Pathway.  So, even though there is no Glucose in the diet, there is an element which enters the Glycolytic pathway during LCHF Meal.

  • Why can’t we reduce both Fat as well as Carbs

The Answer is Simple

Body Needs Energy to Function

This Energy can be got from Carbs or from Fat, but it has to be got from either on of the above (or else there will be protein denaturation leading to Starvation and Finally death)

Because Energy is needed, We have to increase Fat intake when we reduce Carbs Intake

When you can get the energy from both Carbs as well as Fat, why are fats preferred and why not Carbs

The Answer is Simple

When we wat, the energy in our food

  1. Is Spend (or used)
  2. Is Stored

But, Nature has some surprises for you. The two main surprises are

  1. Whatever form of Energy you intake, whatever source from which it is obtained, it is stored as fat. Whether you take Carbs in Excess or Fat in Excess, it is stored as Fat
  2. The amount of Energy that has to be stored and the amount of energy that has to be utilised is determined by the amount of Carbs we intake

In Short,
It is the amount of Carbs in our food determines the amount of fat that gets stored in the body
Let me repeat it again
It is the amount of Carbs in our food determines the amount of fat that gets stored in the body, in the abdomen, inside the blood vessels

So

  • Even if you take Only Carbs and Avoid taking fats altogether, there will be still fat deposition in your body
  • If you taken both carbs as well as fats, there will be fat deposition in your body
  • If you take Low Carbs and High Fats, most of these will be used and very very less will be stored. This is why those with Low Carb High Fat Diet (LCHF Diet) reduce weight and become lean
  • If you take low carbs and low fats, body does not have enough source of energy. Hence body enters Catabolic Phase and the proteins are broken down, denatured and the amino acids are deaminated and they enter the Citric Acid Cycle to give energy. This will lead to muscle wasting,  and left unchecked, even death

Few Questions come up when we discuss Low Carbohydrate High Fat Diet (Paleo Diet, Atkins Diet etc)

But, Dr Mariano Anto Bruno Mascarenhas , my doubt is as insulin is a very important anabolic hormone and plays a very vital role in protein synthesis from amino acids , won’t the decreased insulin also lead onto less protein production and more proteolysis?

You must have known that Insulin is a hormone Secreted by Beta Cells of Pancreas and from There, insulin is directly released into the blood. After getting released into blood, insulin does the following metabolic activities

  1. Increases Glucose Uptake
  2. Increases Glycolysis
  3. Increases conversion of Pyruvate to Acetyl CoA
  4. Increases Glycogenesis
  5. Decreases Glycogenolysis
  6. Decreases Gluconeogenesis
  7. Increases HMP Shunt
  8. Decrease Lipolysis
  9. Increase Fatty Acid Synthesis
  10. Increases TriGlycerides Synthesis
  11. Decreases Ketogenesis
  12. Increase Amino Acid uptake
  13. Protein Anabolic Effect of Growth Hormone (permissive effect)
  14. Increases protein Synthesis
  15. Regulation of Ribosomal Translation
  16. Decrease in Potassium and Phosphorus in Blood
  17. Stimulates Growth and Cell Replication

From this extremely long list, we can know that reduction in blood Glucose is the main function of Insulin. The Main Function of Insulin is Anabolism and Reduction of Insulin is just the side effect of that. 

In fact

  • The number of hormones which reduce blood glucose (like insulin) is quite low
  • There are a huge nunber of hormones which increase the glucose in blood

Why? Because

  • Our Body is inherently not suited to eat Carbohydrates
  • We are inherently suited to eat fats
  • Since we are not supposed to eat Carbs, Nature has given us only insulin to deal with Glucose in Blood and lots of other hormones to deal with fat
  • That is why lack of a single hormone which has to reduce blood sugar leads to disease
  • When one of the hormones which has to increase blood sugar is not functioning properly, the other hormones take the role and hence the person does not become diseased

In Human Evolution, Our Food Pattern Changed to Predominatly Carb just recently

  • That is after we started regular Agriculture and Cultivation
  • That is in the past 2000 years

And, when our body is not suited for Carbohydrates, we take a predominantly Carbohydrate Food. What Will happen

  • Glucose Level in Blood Increases
  • Insulin is the only hormone which has to fight this
  • Insulin Secretion Increases beyond its physiological capacity

What will happen when insulin secretion in kept at a high level for months and years together

Assume a Village with 50 families and a Village Pond which feeds them

The water in the village pond is sufficient for 50 families

What will happen when a Water Bottling Company comes and sets up the factory and uses Bore Well to get all the water

The Pond will become Dry Soon. . .Isn’t it

In the same way, When we eat more Carbohydrates than what we are supposed to eat

  • Lots of Insulin is Needed
  • Lots of Insulin is secreted
  • Over a period of time, the beta cells get tired
  • and
  • At this stage, the patient is given tablets to increase the secretion

After this time, beta cells wear down (the pond is even more drier)

  • We need to give Insulin by Injection

If we don’t eat More Carbs, the amount of Insulin needed is less and our Pancreas and beta cells do not get tired

  • Many people’s pancreas can tolerate only limited Carbohydrates. When this level is crossed, the beta cells cannot cope up
  • Few others have a pancrease which can tolerate more carbohydrates for some time alone, but after that the beta cells get tired and reduce the secretion
  • Few others are blessed with a pancreas that can tolerate any amount of insulin

  • The First Category belongs to Type 2 Diabetes Mellitus (Type 2 DM)

1. Primary Genetic Insulin Resistance
2. Not doing physical work and being the so called couch potato (Sedantary Life Style)
3. Diseases Related to Fat metabolism (Dyslipidaemias)
4. Eating Too Much. (also called Gluttony)

Of these,

If your Obesity and Overweight is due to Sedentary Life Style, you can reduce your weight and waist circumference by Walking, Jogging, Running, Swimming, Cardio, Lifting Weights etc
If your Obesity and Overweight is due to eating too much, you can reduce your weight and excess fat by eating less
If your Obesity and Overweight is due to Dyslipidaemias, you can reduce your weight and excess fat by appropriate treatment

But

If your Obesity and Overweight is due to Primary Genetic Insulin Resistance, the only way you can reduce weight is by taking Low Carbohydrate Diet. Running Miles in Treadmill or Starvation will not solve your problem

  • Why can’t we reduce both Fat as well as Carbs
  • Why we have to reduce only carbs

The Answer is Simple

Body Needs Energy to Function

This Energy can be got from Carbs or from Fat, but it has to be got from either on of the above (or else there will be protein denaturation leading to Starvation and Finally death)

Because Energy is needed, We have to increase Fat intake when we reduce Carbs Intake

When you can get the energy from both Carbs as well as Fat, why are fats preferred and why not Carbs

The Answer is Simple

When we wat, the energy in our food

  1. Is Spend (or used)
  2. Is Stored

But, Nature has some surprises for you. The two main surprises are

  1. Whatever form of Energy you intake, whatever source from which it is obtained, it is stored as fat. Whether you take Carbs in Excess or Fat in Excess, it is stored as Fat
  2. The amount of Energy that has to be stored and the amount of energy that has to be utilised is determined by the amount of Carbs we intake

In Short,
It is the amount of Carbs in our food determines the amount of fat that gets stored in the body
Let me repeat it again
It is the amount of Carbs in our food determines the amount of fat that gets stored in the body, in the abdomen, inside the blood vessels

So

  • Even if you take Only Carbs and Avoid taking fats altogether, there will be still fat deposition in your body
  • If you taken both carbs as well as fats, there will be fat deposition in your body
  • If you take Low Carbs and High Fats, most of these will be used and very very less will be stored. This is why those with Low Carb High Fat Diet (LCHF Diet) reduce weight and become lean
  • If you take low carbs and low fats, body does not have enough source of energy. Hence body enters Catabolic Phase and the proteins are broken down, denatured and the amino acids are deaminated and they enter the Citric Acid Cycle to give energy. This will lead to muscle wasting,  and left unchecked, even death
